Plum Creek Care Center is a cozy skilled nursing facility with a caring home environment. Our outside courtyard and gazebo area provide the perfect place for your loved one to rest and rehabilitate. Plum Creek Care Center has 36 semi-private rooms and 30 private rooms for your convenience.
Rehabilitation services at Plum Creek focus on assisting patients in becoming as independent and safe as they can be in their everyday functional activities. Plum Creek offers Physical Therapy, Occupational Therapy and Speech-Language Pathology to Care Center patients as well as to outpatients.
