The goal of rehabilitation is to assist each patient in becoming as independent and safe as he or she can be in everyday functional activities with the special needs of the geriatric population taken into account.
Rehabilitation Features:
Rehabilitation is offered one to two times a day, up to five days per week based on individual treatment program.
Interdisciplinary team approach for all residents/patients care planning conferences and rehabilitation meetings.
Interdisciplinary team: facility Director of Nursing, Charge Nurse, Social Service personnel, Activities personnel (when appropriate), Dietary (when appropriate), Physical Therapist, Occupational Therapist, Speech-Language Pathologist. For care planning, the family physician and professional consultants are involved.

Continuum of Care:
Our professional rehabilitation team develops functional maintenance programs for those residents who qualify to encourage them to reach their highest potential in the Long Term Care setting.
Lake View Care Center takes the continuum of care to its final level by offering outpatient rehabilitation services.
Patient Profile:
People who have recently experienced an injury, illness or disease with resulting neurologic or orthopedic impairment, general de-conditioning secondary to multiple and/or complex medical diagnoses.
Motivated person, able to work with the Professional Rehabilitation Team to gain a higher level of function requiring lesser care and ultimately, returns home.
Additional Information:
Generally, when the patient has met Medicare criteria for skilled services (physician's order, 3-day hospital stay regarding this spell of illness and requiring the skilled services of a nurse or therapist), PT, OT and SLP are covered by Medicare Part A.
Medicare Part B generally covers 80% of outpatient and long term care rehabilitation services, Supplemental insurance may cover remaining costs. However, this coverage varies from policy to policy.
