24 Hour Skilled Care

Dedicated and professionally trained Registered Nurses, Licensed Practical Nurses and State Certified Nurse Aides provide excellent care to your loved one(s) through 24 hour care. Special services include IV therapies, enteral therapies, and wound care.

Respiratory Service
|
Respiratory therapists offer services to Medicare Skilled residents when appropriate.

Professional Geriatric Rehabilitation Team
|
Occupational, Physical and Speech Therapists offer in-patient screening, evaluations and treatment for all qualified residents.

Restorative Dining
|
Heritage Place offers special feeding and swallowing programs utilizing Speech and Occupational Therapists and Restorative Aides to promote a safe and independent feeding.

Professional Consultants Available
|
Our staff works closely with the Medical Director, Dentists, Dietitian, Pharmacist, Podiatrist, Social Worker, Medical Laboratories, and Enterostomal Therapy Nurses to establish the best care for our residents.

Interdisciplinary Team Approach
|
We specialize in team care planning for our residents by involving the resident family members, physicians, nurses, therapists, department staff and professional consultants.

Alzheimer's Unit
|
Employees of the unit are specially trained to care for the unique needs of residents with Alzheimer's and other related conditions.

Dietary Services
|
We proudly offer our residents a menu of nutritional and delicious choices for their meals which are designed by our registered dietition. Individual menus are planned by our registered dietition with consultation from the attending physician for those with special nutritional needs.

Social Services
|
The Social Services Director provides individual consultations including pre- admission screenings, regular re-evaluation, family support consultation, counseling and discharge planning for your convenience.

Activities Services
|
Heritage Place prides itself on the many activities provided for our residents' entertainment and well being. We hold exercise groups, musical entertainment, bingo, van trips, community outings, social events, monthly birthday parties, educational programs, movies, and bedside activities. Many local volunteer groups visit and offer social opportunities. Worship services and Bible studies are conducted by local clergy.
